Legal Agreement & Consent Form for Therapeutic Massage of the Bare Gluteus Maximus (Bottom / Buttocks)
I confirm and declare that I am knowingly booking and going to receive a therapeutic, non-sexual massage of my bare, naked Gluteus Maximus (Bottom and Buttocks)
I accept, acknowledge and confirm that by signing this agreement / consent form that I fully understand that the massage is a therapeutic, non-sexual massage of my Gluteus Maximus (Bottom and Buttocks) that I am going to receive prior to the treatment being given and that this type of massage therapy treatment is being given for reasons of general health, well-being and wellness thus helping to reduce cellulite in the thighs and bottom / buttocks area to help give a firmer and more toned appearance.
I confirm and hereby give my full consent for Simon to also massage my upper thighs and bare bottom if I am also choosing to book an holistic massage, therapeutic massage, relaxing massage and / or a Lomi Lomi full-body massage therapy treatment too.
I confirm that I have made contact with Simon my professional massage therapist masseur in the full knowledge that I will be booking and receiving a therapeutic massage of my bare, naked, nude Gluteus Maximus (Bottom and Buttocks) and that my massage may also include a therapeutic massage of my thighs and upper legs.
I confirm that I have personally invited / booked Simon who is a professional male massage therapist / masseur to visit my home or hotel room to give me a therapeutic massage of my bare Gluteus Maximus (Bottom and Buttocks) and that I may have to receive the massage treatment when I am nude or when my bottom is bare / exposed with me feeling happy, comfortable to be within my own personal boundaries, limits and comfort zones.
I confirm that I have read through anti-cellulite massage section within Simon's main massage therapy website which is called www.holisticbodymassage.com and I that am booking a therapeutic, non-sexual, firming, toning, massage of my bare Gluteus Maximus (Glutes, Bottom and Buttocks) including a therapeutic massage of my thighs and upper legs to help reduce cellulite in the aformentioned areas.
I confirm that I have read through the website www.holisticbodymassage.com in full which belongs to Simon who is a professional male massage therapist masseur and that I fully understand that booking a firming, toning, anti-cellulite massage may or will most likely involve massage of my upper thighs and bare bottom / buttocks.
I confirm, accept, acknowledege and declare that I have read the section about anti-cellulite massage of the upper thighs and bare bottom within Simon's main website Home Page which is www.holisticbodymassage.com in full regarding professional and therapeutic massage of the Gluteus Maximus (Glute, Bottom and Buttocks) and that I give my full permission with my consent for a therapeutic massage of my bare bottom / buttocks during my appointment booking and massage therapy treatment with Simon.
I hereby give full permission and prior informed consent for my massage therapist / masseur Simon to allow him to give me a therapeutic, non-sexual, firming, toning, massage of my bare Gluteus Maximus (Bottom and Buttocks) to help reduce cellulite in the aformentioned areas.
I also give my full consent for a massage of the bare bottom, buttocks and thighs to be given if I also choose to book a full-body massage, holistic therapeutic Swedish massage, relaxing full-body massage and Lomi Lomi massage.
I do NOT give my permission and consent for any form of sexual massage whch includes massage of my intimate genitalia and I understand that this style of massage will only be given over the undraped bare bottom and thighs.
I understand that Simon will remain completely professional at all times and not deviating from my personal boundaries when giving me a firming, toning, therapeutic anti-cellulite massage or any other massage that includes the massage of my bare bottom and thighs.
I also understand that I may also be completely naked and full nude upon my own private and personal request for an undraped, uncovered naturist massage or I may be fully nude for any of the massage therapies mentioned within Simon's massage therapy website.
Legal Declaration
I hereby sign this legal delcaration consent form to confirm that I am booking a massage therapy treatment with Simon that will include the non-sexual therapeutic massage of my bare bottom to be professionally given with my full permission, approval, knowledge and that I agree to the massage of my bare naked bottom with my informed consent.
By siging this form, I hereby give my full consent and prior permission with informed consent for Simon to give me a therapeutic massage of my legs, thighs and bare bottom / buttocks or massage of my fully naked body when I am completetly nude with pre-agreement, but I only give me consent for a non-sexual, therapeutic massage and I am not giving my consent or permission for erotic sensual massage or massage of an adult sexual nature.
I do NOT consent or agree to anything that might be considered sexual, any sexual stroking or the improper touching of any parts of my body that has been pre-agreed as "off-limits" before or during the actual massage therapy treatment nor do I give my consent to receive massage that is of a sexual nature.
I hereby declare and confirm that I have read and fully agree to the terms of service with my full consent.

I hereby promise and confirm that I NOT transgress any of our pre-arranged boundaries and that I will always respect the pre-arranged boundaries, limits and comfort zones of the aforementioned client who has knowingly booked me to give her a massage therapy treatment of the thighs, bare bottom / buttocks that will always remain within my client's personal, pre-agreed comfort zones, thus making her feel happy, comfortable, safe and secure at all times.
I promise, assure and personally guarantee that all my holistic massage therapies given will be non-sexual and always remain within the pre-agreed boundaries of my female clients and your own personal boundaries, limits and comfort zones.
I promise and state for the legal record that I will NOT go beyond your boundaries and beyond the pre-agreed the boundaries of my client during the course of the massage and that I will remain professional at all times having full respect for my massage client and I confirm that I will always respect wishes / requests of my massage client at all times in accordance with this legal, binding agreement / consent form and in accordance with the client and recipient of any of my massage therapy treatments / services.
I promise that I will always remain faithful and true to all the pre-arranged agreements and that the massage will always remain within your own boundaries and the personal comfort zones / limits of my client, thus enabling you to fully relax, secure and safe in the knowledge that although you are booking a massage of the bare bottom, buttocks and thighs, the massage will NEVER, EVER go byond your own personal wishes, requests, desires or trangress our pre-arranged boundaries.
I guarantee the personal safety, security and privacy of all my discerning female clientele and will always adhere to your own personal wishes / requests during the time spent with me for the full duration of the massage.
I hereby promise and also give my absolute assurances that I would never, ever trangress / cross / go over your own or any of my client's personal boundaries, comfort zones or sexual limits by touching, stroking, stimulating or massaging any part of her female genitalia when giving a massage of the bare bottom or any other type of massage.

This additional service is completely optional and only ever given by individual request, personal preference and personal choice.
There is absolutely NO obligation to ever book a massage of the bare breasts when booking a massage of the bare bottom and thighs and this additional service is always down to the personal discretion of my female clientele and I would only ever give this particular treatment upon request.
